There was another one very similar, but the rear wheel mount plate was different.

Unless you are after a Concours restoration, theses guesses are close enough to get you on the trail again. Most available parts are generic and universal. Most parts for that frame are either universal or fabricated at home or local welding shop. The only thing the original model is good for is finding pictures on line. Replacement parts dwindled up decades ago, but damn, I saw your name, and you already knew all that!
