What does the back side of that look like?

Looks like it makes the timing adjustable (advance/retard).

It does and was ground from a stock cam.

I don't suppose there is any way of getting a pic with the screws out and the gear off the shaft? I am curious how it is made, appears to be a modified gear from one cam added to a modified gear hub from a second shaft. The cam lobes also appear to have been welded up and ground to a new profile.

Modified indeed! Welded up lobes with approximate .380" IN and .390" EX. The cam card is in the box with it.
Not a ton of duration as per build, and never was intended for a stroker.
I still take part in building some here/there from the ground up, (bias to the 5hp Briggs) mostly for other folks now.
