6" is the diameter of the wheel.
My Dad has a 60s Fox Doodlebug, it came with 6" diameter wheels but I think the wheels are only 4" wide.
Tire widths and sidewall heights can differ from tire maker to tire maker, you might be able to fit a 5.5" tire onto a 4" wheel, maybe not.
