Anybody have any clue how to mount this brake?

I have a coyote im restoring, and this brake is giving me some trouble. I dont get what the two forks are for. I also have to make a bracket to weld to my frame, to hold the brake line to my frame.

My project is under my cousins profile "scottessy" labeled coyote project is anyone wants to check out the progress thanks
