Play in Baja Warrior steering stem?

#1
After riding my Baja for several hours, I started noticing that the handlebars wiggled a little more than they should. I tried tightening down the nuts on the bottom side of the top plate of the triple tree to make this go away. However, those nuts were as tight as possible, yet the wiggle was still there. The rubber bushings went flatter over time and space developed. I decided to add some washers in on each side to take up the space and allow proper tightening. The handlebar play was eliminated!

The other day, I noticed that there might be play in the steering stem nut area.... That assembly looks complicated and I don't see how play could have developed over time... I haven't yet checked to see if all the nuts are tight, but if they are and t still has play, do I need to add some washers there also?
