honda engine help

looking at maybe getting this engine and wondering what this bolted on part is

also are these mortar mixer engines tapered shaft? or not always?

I believe that's a gear reduction unit, and the shaft coming out of it should be straight.

how does a gear reduction box work? would i need to take that off for use on a gokart?

Well, there is a small gear cut directly on the end of the crank which drive a larger one cut on the output shaft (what you see sticking out of the box with the belt pulleys on it), so you couldn't take the box off and still use the engine without installing a longer crankshaft. You can use the engine with the reduction box, but you'll need a clutch with a lower engagement RPM setting and a lower gear reduction from the clutch to the wheel.
